DVA Community Nursing

How Crisden Care can help

Crisden Care is an approved provider of DVA Community Nursing services and we are proud of the relationship we have with the Department of Veteran's Affairs, enabling our team to assist eligible Veterans and their widows. 


The objectives of DVA's Community Nursing Program are to enhance the health outcomes and independence of the eligible person by avoiding early and unnecessary admission to residential care and/or hospital by providing access to community nursing services that meet their assessed needs.

 

The community nursing program can assist with:


  • Medication Management
  • Symptom Management
  • Continence Management
  • Wound Care
  • Diabetes Management
  • Personal Care, including assistance with dressing and showering


Program services are determined by your needs and may be episodic or ongoing in nature based on your assessed needs by DVA.

 

Eligibility


Depending on your DVA card type, the department will fund certain types of care. Only gold and white card holders are eligible for the program, unless permission is received from the DVA with your referral for services.


Gold Card holders: 

DVA will fund community nursing services that meet your assessed personal care and / or clinical requirements.


White card holders: 

DVA will fund community nursing services if they are required due to an accepted disability, this includes war or service caused injury or disease.

Referrals

All DVA Community Nursing services require a written referral from an authorised referral source. This may be:


  • A treating medical practitioner in a hospital.
  • Hospital Discharge Planner. 
  • Nurse Practitioner specialising in a Community Nursing field.
  • General Practitioner (GP).


The referral should be made on the designated referral form, which can be found below for download. Please email completed referral forms to referrals@crisdencare.com.au
